How to Make Fatal Error Bug
1. Buy a Courier and use it.
2. Buy a Crow recipe and give it to the Courier.
3. Buy Ring of Protection then drop it.
4. Buy Sobi Mask then drop it a bit far from the Ring of Protection.
5. Use the Shift button/queue command on the Courier to make him pick the Ring of Protection then the Sobi Mask then transform to Crow.
(place your Courier a bit far from those items, hold Shift and then right click on the Ring of Protection, right click on the Sobi Mask and left click on the change into Crow button, release Shift)
6. To know if it is successful, the Courier will turn into Crow but the Ring of Basilius is not formed.
7. Build Armlet of Mordiggian, activate it then give it to the Crow.
8. Last step, once the Armlet of Mordiggian already on the Crow, click it and BOOM.. Fatal error will occur to all the players at the game.
This bug is tested on both 1.24d & 24e patch and i assume also work on previous patch. Thanks to body_****@yahoo.com who telling me this Dota 6.66b Bug (contact me if you don't want your name here). I hope IceFrog will include a fix of this bug on the new map, either on Dota 6.66c or maybe on Dota 6.67. Enjoy!

i alwiz get the error message even when im not doing the things stated that could trigger the fatal error...alief help to dig more bout this issue bcoz its damn frustrating....
yupe the fatal error is damn frustrating...it much worse if ur playing the ai maps...
i thought item bugs can only make pudge/vs take blink dagger, now even can make game crash =.=
icefrog really need to recode the item scripts
this is funny...
even though it costs a bit much, surely the team would have enough money to use this bug when they are losing, team members may contribute pots to be sold by the one using the bug..
it is really annoying, hope it will be fixed at the next version because it is game ENDING
Whenever i play with URSA in 6.66b AI, the game always crashes in middle of match with above mentionned fatal error. But when i play with LINA this does not happen. And game goes no smoothly. What is this? Try it. Does it happen to u?

yes when using ursa warrior or POTM especially....the game tends to crash!!!!! argh...damn frustrating..
